What does "Evidence of Insurability" mean?

If you enroll in Optional Term Life Insurance or Dependent Life Insurance, you may be subject to a Statement of Health, also known as Evidence of Insurability. These terms are used interchangeably. This is a form that you are required to complete and submit to the life insurance administrator. Remember, increased amounts of life insurance that are subject to a Statement of Health will not be reflected in your current coverage until approved.
