	Dental

Dental benefits provide comprehensive care for preventive, general, prosthetic dental care and orthodontic care. CITGO offers a dual choice dental preferred provider plan through MetLife. You will have the ability to choose from two dental options:

Option 1 - Dental Basic Option
Option 2 - Dental Plus Option

The dental plan is a stand-alone plan and not part of the medical plan. MetLife is the claims administrator for the dental plan and you have access to a network of private-practice and dental group providers. Benefits are higher when you use a network provider, however, the out of network benefit offers flexibility to see your choice of provider.

	Vision

Vision benefits provide coverage for routine eye examinations and glasses (frames and lenses) or contact lenses. CITGO provides you access to a dual choice vision program which includes:

UnitedHealthcare Vision has updated their contact lens formulary. Choosing from the available selection of covered materials can help maximize your contact lens benefit. Please consult with your eye provider to decide which product is best for you.


	Life, Accident & Disability

CITGO offers access to optional insurance programs for income replacement due to death or non-occupational accidental injuries. These programs provide financial protection for you and your family. Your options include:

Employee Optional Life
Spouse and Child Optional Life
Personal Accident with Employee Only or Family Coverage

Effective January 1, 2022, Securian Financial will replace MetLife as the new life and accident insurance administrator. Refer to the Salaried or Hourly Benefits Brochure for additional information.


	Employee Assistance Program

The Employee Assistance Program (EAP) and WorkLife Benefit is designed to provide confidential support for those everyday challenges as well as serious problems. It’s available around the clock — anytime you need it. Eligible plan participants can use access code 42920 to access program benefits.
