Will I ever need to submit my own claims?

Certain claims will still need to be submitted to United Healthcare including Medicare Part A claims. If there are remaining expenses to be paid to the provider, please submit the Medicare Explanation of Benefits (EOB) and the United Healthcare Medical Reimbursement Claim Form to the address on the claim form. If you would like United Healthcare to pay the provider directly, please indicate this on the claim form. If you do not, tell United Healthcare to pay the provider, and the reimbursement will go to you, not the provider. You may obtain a United Healthcare Medical Reimbursement Claim form by printing one from www.myuhc.com , or by calling United Healthcare’s Customer Service Unit at 1-866-317-6359.
